Studies on Urea, Electrolytes and Water Metabolism of Brain in Acute Uremia


Abstract
The complex metabolic events which characterize uremia are associated with clinical manifestations of diffuse involvement of the central nervous system. These include the full spectrum of organic brain syndromes, progressing from mild impairment of intellect to coma; extrapyramidal syndromes manifest by abnormal involuntary movements, tremors, rigidity, and asterixis increased neuromuscular irritability and lowered seizure threshold. While early studies reported a variety of neuropathological findings, the more recent one indicates that these represent nonspecific changes. There. is often histological evidence of neuronal degeneration which is indistinguishable from that seen in anoxia. Clinical experience with the artificial kidney has established that toxic factors responsible for uremic encephalopathy are readily dialyzable, and thereby the neurological manifestation of uremia may be reversed rapidly.
Despite sophistication in :the management of renal insufficiency, the pathophysiology of encephalopathy is poorly understood, and analyses of brain with traditional neuropathological techniques have failed to yield any specific structural correlations. Therefore, an experimental model of uremia was established in rabbits in order to characterize physiologically the encephalopathy in terms of several aspects of brain metabolism.
On experiments 60 rabbits weighing 1.5-2.5kg -15 were normal control group, 15 were uranium induced acute uremia group, 15 were uranium induced acute uremia group with tap water administration of 50ml per kg prior to experiment, 15 were uranium induced acute uremia group without water prior to experiment were studied.
The brain slices were made for analysis of urea, sodium, potassium and water.
The results were as follows:
1. The plasma osmolarity and potassium were increased but the sodium was decreased in acute
~mic rabbits than the normal ones.
The water content was no significant difference between acute uremic rabbits and the normal
PS.
>. The urea amount was markedly increased in acute uremic rabbits brain than the normal ones.
1. The sodium content was significantly decreased in acute uremic rabbits brain than the normal ones. i. The potassium content was no significant difference between acute uremic rabbits and the normal ones.
